Forms and procedure for submitting documents

1. A certificate of education or a period of study confirming the development of an educational program of higher medical education in the scope and in the specialty (field of training) – for students / A specialist diploma (bachelor's degree) in the specialty (field of training) – for persons with higher medical education.

2. A copy of the identity document (a copy of the passport, 1-2 pages).

3. An application filled out according to the sample (to be filled out in room 321).

4. 3 sheets of clean A4 paper.

5. File

The documents are submitted in a file at:
